Student Accidents
The Accident Report Form for Student Accidents is completed by the instructor/staff person in charge of the student who has been injured in the classroom or on campus. All Student Accident Forms are sent to David Tabor (Safety Officer) and Vanessa Owen in Student Services for student insurance processing.

The Investigation of Occupational Injury form is used by employees of the college to report a work related accident or injury. All Investigation of Occupational Injury forms are sent to Tommy Oakman and David Tabor.
